Hi. Do I need to backup any files before I install a car mod that uses a self installer.

No one will answer you giving 100% guarantee because selfinstallers are blackboxes which may do a lot of things not only with the game installation but event with the system.
If you are not sure, you can install mod in a temporary directory, then check what files are installed.
It may help to prevent overwriting existing files, but not indicate if some files may be deleted by installer.
